Funding

Monitoring requirements

Chemical dependency

  1. Length of contract: 5 years
  2. Random urine drug screen frequency:
    • Year 1: 1 time per week
    • Year 2: 1 time per month
    • Year 3: 1 time per quarter
    • Year 4: 1 time per quarter
    • Year 5: 1 time per quarter
    • To completion: 1 time per quarter
  3. Support (self help) group requirements:
    • AA
    • NA
    • Caduceus
    • Professionally facilitated: if available group; with individual
  4. Support (self help) group frequency:
    • Year 1: 3 times per week
    • Year 2: 3 times per week
    • Year 3: 2 times per week
    • Year 4: 2 times per week
    • Year 5: 2 times per week
    • To completion: 2 times per week
  5. Therapy or treatment requirement: All physicians to comlete appropriate ISMA approved Tx plus follow-up group/individual/marital program
  6. Work or practice monitor requirement: as needed
  7. Other provisions: Candace generally meets with partners/office staff as part of return to work - to educate on illness, monitoring roles

Mental health

  1. Length of contract: 2 years, variable
  2. Support (self help) group requirements: if diagnosed with chemical dependency, follow above guidelines plus meds/psychiatric follow-up
  3. Support (self help) group frequency:
  4. Therapy or treatment requirement: psychiatric and therapeutic monitoring, usually weekly for counseling; monthly to quarterly for medcations
  5. Work or practice monitor requirement: as needed
  6. Other provisions:
