No Program services Types of disease, illness, or conditions monitored: Chemical dependency Mental health Behavioral health problems Sexual misconduct and/or boundary violations Physical illness Other: Available for referrals for marital, stress, etc. Services provided to which populations Physicians - MD Physicians - DO Families of physicians Dentists Residents  Funding Please indicate the primary sources of funding for your program: State medical society Hospital and private contributions Participant fees: $75 per month for members, $125 per month for non-members Monitoring requirements Chemical dependency Length of contract: 5 years Random urine drug screen frequency: Year 1: 1 time per week Year 2: 1 time per month Year 3: 1 time per quarter Year 4: 1 time per quarter Year 5: 1 time per quarter To completion: 1 time per quarter Support (self help) group requirements: AA NA Caduceus Professionally facilitated: if available group; with individual Support (self help) group frequency: Year 1: 3 times per week Year 2: 3 times per week Year 3: 2 times per week Year 4: 2 times per week Year 5: 2 times per week To completion: 2 times per week Therapy or treatment requirement: All physicians to comlete appropriate ISMA approved Tx plus follow-up group/individual/marital program Work or practice monitor requirement: as needed Other provisions: Candace generally meets with partners/office staff as part of return to work - to educate on illness, monitoring roles Mental health Length of contract: 2 years, variable Support (self help) group requirements: if diagnosed with chemical dependency, follow above guidelines plus meds/psychiatric follow-up Support (self help) group frequency: Therapy or treatment requirement: psychiatric and therapeutic monitoring, usually weekly for counseling; monthly to quarterly for medcations Work or practice monitor requirement: as needed Other provisions: Please describe any other monitoring services provided:
