Monitoring requirements

Chemical dependency

  1. Length of contract:
    • 3 years
    • Other: Clinical or licensing board requirements may deem 5 years or longer
  2. Random urine drug screen frequency:
    • Year 1: 2 times per week (for 3 months)
    • Year 2: 1 time per week
    • Year 3: 1 time per week
    • Year 4: 1 time per week
    • Year 5: 1 time per week
    • To completion: 1 time per week
  3. Support (self help) group requirements:
    • AA
    • NA
    • Caduceus
      NOTE: One meeting per week must be a physician support group
  4. Support (self help) group frequency:
    • Year 1: 3 times per week (first 3 months)
    • Year 2: 1 time per week
    • Year 3: 1 time per week
    • Year 4: 1 time per week
    • Year 5: 1 time per week
    • To completion: 1 time per week
  5. Therapy or treatment requirement: On a schedule determined by a therapist who will submit quarterly reports for the duration of the contract
  6. Work or practice monitor requirement: Monitor and chief of service required who regularly meet with physician and who will submit quarterly reports for the duration of the contract
  7. Other provisions:
    • Physical exam within 60 days of contract
    • Monthly meetings with associate director and quarterly reports for the duration of the contract

Mental health

  1. Length of contract:
    • 2 years
    • Other: Minimum of two years, variable on a case-by-case basis, referred to as "Behavioral Health"
  2. Support (self help) group requirements: Other: Support group approved by director, once monthly
  3. Support (self help) group frequency:
    • To completion: 1 time per month
  4. Therapy or treatment requirement: Individual or group therapy required with therapist of record who will submit quarterly reports for the duration of the contract, frequency once weekly for six months and then on a frequency determined by the therapist.
  5. Work or practice monitor requirement: Monitor and chief of service required who regularly meet with physician and who will submit quarterly reports for the duration of the contract
  6. Other provisions: Monthly meetings with associate director and quarterly reports for the duration of the contract
  7. Please describe any other monitoring services provided: as needed, committee monitor (quarterly), worksite monitor, therapeutic monitoring groups, hair analysis, naltrexone
