Monitoring requirements

Chemical dependency

  1. Length of contract: 5 years
  2. Random urine drug screen frequency:
    • Year 1: 2-3 times per month
    • Year 2: 2 times per month
    • Year 3: 1-2 times per month
    • Year 4: 1-2 times every 3 months
    • Year 5: 1 time every 3 months
    • To completion: 1 time every 3 months
  3. Support (self help) group requirements:
    • AA
    • NA
    • Caduceus
    • Relapse prevention counseling, if needed
  4. Support (self help) group frequency:
    • Year 1: 4 times per week
    • Year 2: 3 times per week
    • Year 3: 3 times per week
    • Year 4: 3 times per week
    • Year 5: 3 times per week
  5. Therapy or treatment requirement: If relapse group required - both individual and group
  6. Work or practice monitor requirement: Meet with monitor 2 times per month
  7. Other provisions:

Mental health

  1. Length of contract: 1-5 years, depending on diagnosis
  2. Support (self help) group requirements:
    • Caduceus
    • Individual therapist
    • Caduceus/Group for dual dx
  3. Support (self help) group frequency:
    • Year 1: 4 times per week
    • Year 2: 3 times per week
    • Year 3: 3 times per week
    • Year 4: 3 times per week
    • Year 5: 3 times per week
    Note: Caduceus/group for dual diagnosis only
  4. Therapy or treatment requirement: 3-4 individual sessions per month
  5. Work or practice monitor requirement: Therapist may serve as monitor but sometimes separate, independent mentor/work monitor meetings 2 times monthly
