Monitoring requirements

Chemical dependency

  1. Length of contract: 5 years
  2. Random urine drug screen frequency:
    • Year 1: 1 time per week
    • Year 2: 2 times per month
    • Year 3: 2 times per month
    • Year 4: 1 time per month
    • Year 5: 1 time per month
    • To completion: 1 time per month
  3. Support (self help) group requirements:
    • AA
    • Other: Individual/family therapy as needed or in rural areas (for accountability) on case-by-case basis
  4. Support (self help) group frequency:
    • Year 1: 3 times per week
    • Year 2: 3 times per week
    • Year 3: 3 times per week
    • Year 4: 3 times per week
    • Year 5: 3 times per week
    • To completion: 3 times per week
  5. Therapy or treatment requirement: Formal C.O treatment required for advocacy unless two years abstinence documented
  6. Work or practice monitor requirement: As recommended by treatment center
