PLEASE JOIN US: The FSPHP would like to invite you and your organization to join other leaders in the field who have made a decision to become sponsors of the 2019 Federation of State Physician Health Program's (FSPHP) Annual Education Conference. Our annual conference is attended by over 250 state physician health program administrators, care providers, and allied health professionals dedicated to the purposes of the FSPHP and its member state programs. This is a unique opportunity to network with professionals dedicated to issues of physician well-being, including the treatment of substance use disorders and mental health problems facing physicians and other licensed health care professionals in the United States, Canada and other parts of the world.

Please review the exhibitor levels, recognize the benefits of each, and consider the highest available level for your organization. FSPHP appreciates your sponsorship of this conference toward the mission of FSPHP. The conference offers ample exhibitor time with attendees, including a welcome dinner, silent auction, breaks, breakfast, exhibitor introductions and networking opportunities during lunches. The conference will open with lunch offered within the exhibit hall where all refreshments and catering is offered. Exhibitors may offer door prize giveaways by collecting business cards from attendees and drawing a winner. We have added time on the agenda during the last day of the conference for each exhibitor to present any winners. All education sessions are open to our exhibitors to further their learning with our attendees. At the request of our exhibitors, FSPHP will once again offer a game to give attendees incentive to visit your exhibits. FSPHP will continue with an event mobile application allowing for interaction with attendees and listing all exhibitors by sponsorship level and alphabetically for easy look-up.


WHO ATTENDS: The program is designed for physicians of all specialties, physician assistants, attorneys, program developers, physical therapists, case managers, nurses, licensed social workers, toxicologists, scientific researchers, and clinical coordinators involved in physician and professional health programs.


OPENING NIGHT SILENT AUCTION DINNER, Wednesday, April 24, 6:00 pm - 8:30 pm: FSPHP will feature a Silent Auction at our Opening Night Dinner, in the Exhibitor Hall for all conference attendees and exhibitors. Proceeds will benefit the Federation of State Physician Health Programs’ mission to support physician health programs in improving the health of medical professionals, thereby contributing to quality patient care. FSPHP MISSION: The FSPHP mission is to support physician health programs in improving the health of medical professionals, thereby contributing to quality patient care. The FSPHP annual education conference provides a unique forum for education and exchange of information among state physician health programs. Through this program, PHPs further their efforts to develop common guidelines and enhance the awareness of issues related to physician health, thereby improving the ability to support the health and well-being of the profession.

Transportation: 
To the hotel, Dallas/Fort Worth International Airport (DFW) is about 25 miles and Dallas Love Field (DAL) is approximately 35 miles. Taxi and shuttle service are available from both airports. Cab fare from the Airport to The Renaissance Worthington is approximately $65. 

Set Up and Breakdown: 
~Early registration and set up are available on Tuesday, April 23, 2019 from 4:00pm - 6:00pm. 
~Additional registration time is Wednesday, April 24, 2019 from 9:00AM – 10:30AM. 
~Upon arrival at hotel, you will be responsible for tracking your material. 
~Breakdown time will be at 12:00PM on Saturday, April 27, 2019. 
~Please provide any special requirements that are needed to set up your booth at the conference on registration form. 
~Each exhibitor will be provided with one 6-foot draped table and one chair. 
~All educational sessions are open to exhibitors to attend for ample networking opportunity. 

Shipping: 
~The group name should be referenced on shipped exhibit materials as “The Federation of State Physician Health Programs” with your company/organization and representative names. 
~All returns need to be pre-labeled with a scheduled pick up time. Exhibitors are responsible for their own returns. 

Required Certificate of General Liability Coverage: 
In accordance with the FSPHP’s General Liability Coverage Policy, it is required that exhibitors submit a certificate of Commercial General Liability Coverage, naming Federation of State Physician Health Programs as an additional insured on your organization’s policy, written with a single limit of $1,000,000 per each occurrence and an aggregate limit of at least $2,000,000. This is something that your insurance provider can typically provide to you. 

Your Certificate of Insurance in naming FSPHP as an additional insured should read: 

RE: 2019 FSPHP Annual Education Conference, Fort Worth, TX April 24-27, 2019 Federation of State Physician Health Programs (FSPHP) is listed as an Additional Insured on the General Liability with regard to the above referenced meeting.
